== Clinical uses of C-peptide testing == Patients with diabetes may have their C-peptide levels measured as a means of distinguishing type 1 diabetes from type 2 diabetes or maturity-onset diabetes of the young (MODY). Measuring C-peptide can help to determine how much of their own natural insulin a person is producing as C-peptide is secreted in equimolar amounts to insulin. C-peptide levels are measured instead of insulin levels because C-peptide can assess a person's own insulin secretion even if they receive insulin injections, and because the liver metabolizes a large and variable amount of insulin secreted into the portal vein but does not metabolise C-peptide, meaning blood C-peptide may be a better measure of portal insulin secretion than insulin itself. A very low C-peptide confirms Type 1 diabetes and insulin dependence and is associated with high glucose variability, hyperglycaemia and increased complications. The test may be less sufficient to diagnose or recognize a subgroup of type 1 diabetes named Latent autoimmune diabetes in adults (LADA), whose C-peptide levels may not be as low as those in typical Type 1 diabetes while may sometimes overlap with those seen in type 2 diabetes, and Beta-cell antibody testing is needed for better diagnosis in this case. C-peptide can be used for differential diagnosis of hypoglycemia. The test may be used to help determine the cause of hypoglycaemia (low glucose), values will be low if a person has taken an overdose of insulin but not suppressed if hypoglycaemia is due to an insulinoma or sulphonylureas.

== Signs and symptoms == The various types of VWD present with varying degrees of bleeding tendency, usually in the form of easy bruising, nosebleeds, and bleeding gums. Women may experience heavy menstrual periods and blood loss during childbirth. Symptoms of VWD vary depending on age, sex, and VWD type. In children, bruising and nosebleeds are common symptoms. In adults, easy bruising, heavy menstrual bleeding, and bleeding from minor wounds are more common. Sixty to eighty percent of people with VWD have excessive bleeding after surgery or dental extractions. Gastrointestinal bleeding from dilated blood vessels lining the gut (angiodysplasia) can result in severe, lifethreatening gastrointestinal bleeding. This most commonly occurs in seniors with type 2 or 3 VWD. Von Willebrand factor levels normally increase with age, so disease severity often decreases with older age. In women with VWD, 80% have heavy menstrual bleeding with 20% requiring removal of the uterus (hysterectomy). Due to its association with heavy menstrual bleeding, VWD is more commonly diagnosed in women. Women with VWD may also experience heavy bleeding after delivery (postpartum bleeding). ==== Neutrophil extracellular traps ==== In 2004, Brinkmann and colleagues described a striking observation that activation of neutrophils causes the release of web-like structures of DNA; this represents a third mechanism for killing bacteria. These neutrophil extracellular traps (NETs) comprise a web of fibers composed of chromatin and serine proteases that trap and kill extracellular microbes; thus, by forming NETs (NETosis), neutrophils can bind, disarm, and kill microbes independent of phagocytic uptake. These functions are achieved through the release of highly concentrated antimicrobial components including proteins from granules and powerful histone proteins from the nucleus. In addition to their possible antimicrobial properties, NETs may serve as a physical barrier that prevents further spread of pathogens. Trapping of bacteria may be a particularly important role for NETs in sepsis, where NETs are formed within blood vessels. Finally, NET formation has been demonstrated to augment macrophage bactericidal activity during infection.
